1. Bellsouth employees must visit http://my.bls.com/hrplus/partnership/ for RF-1806. Form can only be accessed via BellSouth intranet.
   
   
2. Directions to form from BellSouth's Home Page: Click on HR Plus tab,then PARTNERSHIP/Educational Assistance link and forms or contact PARTNERSHIP at 888-507-2287.
   
   
3. Student must write "CISCO TRIAL" across top of form
   
   
4. For approval, complete form and mail to the PARTNERSHIP/EA office at BellSouth Center 675 W. Peachtree St. NW, Suite 20U80, Atlanta,GA 30375.
   
   
5. Once application is approved, a voucher will be faxed to CGTC and the student will be mailed an approval letter if using the prepay process.
